FORM A (NEW APPLICATION)

As a new applicant to IROCC, this form will need to be completed in full, printed, signed by participant and participant's department representative and mailed to IROCC along with any requested documentation and a check or money order.

Supporting Document Upload

The following documents are required as part of the IROCC concealed carry application process.

  • F.O.I.D CARD
    Applicants must submit a photocopy of their valid Illinois Firearm Owners Identification Card.
  • RETIRED DEPARTMENT PHOTO ID CARD
    Applicants must submit a photocopy of the photo identification card issued by law enforcement department/agency from which they retired/separated.
  • PHOTOGRAPH FOR QUALIFICATION CARD
    Applicants Must submit a color photograph that will be used on their new retired officer firearm qualification card.
