MTU Information for Hosting Shoots

Please note: All forms are provided using Adobe Reader, which allows information to be input directly onto the forms. While this will enable you to complete the form on-line, you will not have the ability to save documents.

Forms to Submit to IROCC when hosting a Certification Shoot:

IROCC Individual Score Sheet - Complete and return one roster for each retiree attending the Qualification Shoot

IROCC Reimbursement Form - Complete & return one form for each Qualification Shoot

MTU's are asked to notify the IROCC office of the dates of their IROCC qualification shoots no less than 60 days in advance. This allows office staff to provide the pre-qualified retired officers with the most up to date range schedule.  This information can be emailed to irocc@letac.org.

Please also be sure to include any notes regarding your qualification shoots when submitting the roster and score sheets to the office.  Staff needs to be made aware of any possible issues or problems so that notations may be made in the individual's file for future reference.  Your assistance in this is greatly appreciated.
